0000159: Access Denied - Session and Other reasons

Description

Getting access denied when trying to submit a form is un informative for a user and means nothing.

This needs to be handled with the error system.

If its a session time out. Flashing the error "Your Session Timed out please try again" would be more informative.

Also for Cross domain submissions: "This Request was submitted from a different website please check your web address" etc?
